The failures follow a known pattern: data was not ready, nobody defined what success would look like, and tools were layered on top of old workflows. This research did not design our product. It designed our engagement: staged, measured, and run by an external team whose only job is to make the layer work.

In the same MIT study, external implementation partners succeeded at roughly twice the rate of internal builds. Correlation is not causation. We simply like those odds better when implementation is someone's whole job.
